Good initiative :-)
I have a small addition to #5 on the list (let's call it 5a, and a new
sub-point (5.b)):
5a. If you want to set cookies with setCookie(), output header
information with header() or equal, ensure that you have not outputed ANY
characters. That includes whitespace before your starting <? or <?php
too.
5b. If you really do need to output data before setCookie() or header()
(a conditional redirect is one example), then use the output buffering
functions (ob_start(), ob_end_clean(), ob_end_flush(), ob_get_contents(),
ob_implicit_flush()).
