Ok, here are the lines you should have (comment out the rest):
LoadModule php4_module modules/libphp4.so
AddModule mod_php4.c
Make sure that modules/libphp4.so exists (path is relative to the apache server
dir).
If libphp4.so doesn't exist, then you should replace that line with
LoadModule php4_module lib/apache/libphp4.so
Usually modules are in libexec dir, but I don't know how your apache was
compiled.
