The person who posted the message you refer to, who by the way is
something of an idiot, actually renowned as such in certain circles,
eventually figured out that someone else in the office had set
file_uploads to off on that development server in response to earlier
security issues, which causes the silent discarding of multipart/form-data
forms.
He spent many hours in the debugger watching the data travel through
Apache and PHP before discovering this, and occasionally still kicks
himself.
