Just a quick additional note: post_max_size actually needs to be slightly higher than
upload_max_filesize, as it must allow for HTTP headers in addition to the actual file itself (plus,
of course, the size of any other POST fields in the same form!). The difference probably only needs
to be a few hundred bytes, but with upload_max_filesize=200M I'd probably take the easy way out
and use post_max_size=201M!!!
