note 45671 added to install.windows

Oh my God!!! I've been hacking away at this stuff for three days and finally got it! 'ungdi at hotmail dot com', bless you. Your note here about the 'DefaultAppPool' saved my sanity. I ended up going with the 'Local System' setting because the 'IWAM_servername' account wouldn't work. I still don't know why either. The passwords were correct and they had full control to everything just to rule out permissions while I was testing. I'm running Windows 2003 Server with IIS 6 and it just didn't like it unless I used 'Local System' for 'DefaultAppPool'.
