I needed to set additional access to my php program directory. This may be because I don't allow Anonymous Access to my website. I added Read Write permissions for IIS_WPG group and my allowed users to the php program directory. Not exactly sure why, but it worked after much hair pulling. Also, as noted elsewhere, I could run as Script only instead of Scripts and Executables.
