I wrote a simple class called OpenQuiz here:

It allows for easy authoring of multiple choice quizzes, true/false
questions, scoring, redirection upon success. There is also an
object-oriented JavaScript version for Kiosk applications where a server is
unavailable. And I was thinking of extending it to do multi-page forms.
There is no database, and no 'backend'. It is simply for putting a quiz on
a page.
