Howdy Walt,
This is from the Smarty compiling its templates, if you run things once as root it should fix things. For 2.0 were going to be moving to a non-compling template to fix this problem. The other work around is to copy the template directory so some place that is writable and then set "templatebase" (--templatebase=/writabledir) to the new directory.

IMHO this tool should not try and write/touch files in the pear installed dir, because this prevents anyone other then someone w/ write privs to the pear install dir to even run phpdoc....as I am seeing in this case.
