I do an automated install of pear, and I haven't run into issues yet:
C:\test\php install-pear-nozlib.1_4_9.phar -b "C:\test" -d
"C:\test\lib\pear" -p "C:\test\php.exe"
-b is the php dir
-d is the pear install dir
-p is the php executable path
