QFC: form with external parameter, managing the defaults

I've been confronted to the following problem:
- starting QFC with an external REQUEST parameter, and loading the defaults
accordingly
- when the user abandons the form mid-way (for example with the back
button), the defaults remain set, even though she now chooses another
REQUEST parameter
I have assembled a simple example case with a way to address this problem.
