Release notes

* Fixed a bug in PHPUnit2_Framework_Assert::assertEquals().
* Fixed declaration of PHPUnit2_Runner_StandardTestSuiteLoader.
- Removed Xdebug / Code Coverage code because (although working) it
is not usable within the PHPUnit2-2.0.X release family.
