HTML_Quickform problem

Hi,
I'm having problems with getting HTML_Quickform to run "out of the box".
I've installed the pear package in /usr/share/pear and have the base packages, but I get the
following error when I run a very simple test script:
Fatal error: Cannot instantiate non-existent class: html_quickform_header in
/usr/share/pear/HTML/QuickForm.php on line 566
My test code was:
require_once "HTML/QuickForm.php";
$form = new HTML_QuickForm('frmTest', 'get');
$form->addElement('header', 'MyHeader', 'Testing QuickForm');
$form->display();
Any suggestions/help?
