Have you tried to make some output before the require_once()?
Can you see that? Have you also tried to set your error_reporting to
E_ALL (ini_set('error_reporting','E_ALL')) to see all error messages?
Have you confirmed that the directory you´re trying to add to your
include_path is correct? You can do so by letting a script output the
content of $_SERVER['DOCUMENT_ROOT']. If your webserver is an IIS, try
to put a script in the topmost directory of your web account and output
dirname(__FILE__) to get the absolute path of the document root.

Markus
