Sorry my fault.
I need to update the online documentation with the version from CVS.
I changed the way includes are handled in MDB.
You now only include the MDB.php and all other files are included
automatically (like the lob support). Other classes like MDB_Date and
the MDB_Manager are included using MDB::loadFile(). This change was made
because of the limitations of the require_once method that can get
confused if you include the same file with different relative paths.
So the first part of your script needs to look like this:

require_once('MDB.php');
MDB::loadFile('Manager');
...
