pear 1.2.1 is not very sophisticated when it comes to installing things. The closest command to what you need is upgrade-all, but this will fail if you installed dependencies out of order. The PEAR that is in CVS fixes this and also adds a few handy command-line switches, --alldeps and --onlyreqdeps which install alternately all dependencies of a packages, and only required depdendencies.
You still have to specify the path to a file in order to install without downloading, unfortunately, and pear does not expand wildcards. I will talk to other core devs to see if they would be in favor of this expansion or not, as it would be relatively simple to implement.
