You're certainly not the only one having troubles with the ISAPI module. On my setup it causes the IIS to send out 500 server error pages at the slightest provocation and on the simplest of scripts. I've tried attaching the VC++ debugger and single stepping but have been unable to get anywhere near to understanding what's going wrong. If you can get anything going wrong which is reliably repeatable, please let us know, and if you have time, inclination and VC++, try using the debugger and see if you can find any clues.
