Obviously his c-client installation is buggy. If he has latest imap-2001 package, he also should have imap4r1.h there.
I copied manually imap4r1.h, I still get the same problem. Only 1 point to prove there's at least a regression from 4.0.6: this setup does work with PHP 4.0.6. Plus, it does work (4.1.0RC3) when specifying some lib dirs bit not others (see below, original mail), so, it passes test somehow.
