Compiling GD into PHP4 under Windows

I have been struggling to get GD included in the build for Win32 - The way I figure it could either be compiled in as part of the source tree or as a module. The module is probably the smarter choice, but I have had no success in either case.
I'm sure everyone is busy doing other things, but perhaps someone has had some success in this area? Maybe some general info on compiling these extra modules in? As an aside, I'd also like to be able to use GD1.3 (which I have).
