I too had a bit of fear about using the 4 beta in production. But I'm very happy with how it has worked out for rejuvenation.com (which would be perhaps a bit smaller than one of the "medium to large companies"). PHP gave me a great deal of flexibility in creating a translation of files used by our off-line product configuration software, as well as a product "wizard" (that helps determine which product is right by the customer's answers to a few questions).
