Yup, I had the same error (access violation in 'inetinfo.exe') after
installing php4.0b3 under IIS 4.0 / NT4sp3. IIS refused to work until I
removed the isapi-reference from its configuration... the dll seemed to
load okay (as seen from the IIS Console, although it had an 'unknown'
priority). But despite that, it didn't work.
