I had the same problem under IIS: "access violation in 'inetinfo.exe'".
Then suddenly after restarting the machine (not only IIS) it worked but
i got the fatal error "undefined function in 'local.inc'". I removed
the ISAPI filter from IIS, restarted the server (w/ "net stop
iisadmin" and then "net start w3svc"), applied the ISAPI filter again
and restarted the server once more.
The problems seem to have gone... :)
