The PEAR stuff respects --prefix, it does for me:
$ ./configure --prefix=/opt
...
$ make -n install
make install-exec-am install-data-am
make[1]: Entering directory `/home/tjafs/ssb/cvs/php4/pear'
make[1]: Nothing to be done for `install-exec-am'.
/bin/sh ../mkinstalldirs /opt/lib/php /opt/lib/php/DB && \
/usr/bin/install -c -m 644 ./DB.php /opt/lib/php && \
/usr/bin/install -c -m 644 ./DB/odbc.php /opt/lib/php/DB
make[1]: Leaving directory `/home/tjafs/ssb/cvs/php4/pear'
Check your pear/Makefile and see what "prefix" is set to there.
