I've had similar baffling problems getting php going on an NT box at one of my ISPs. Nothing produced any error messages, but php just wouldn't work from the web server. In the end, it turned out that IUSR_SERVERNAME did not have sufficient privileges to get php to execute. Because the ISP did not want me to change the permissions, I created another user, 'phpuser' which was a member of 'users' and configured iis to use my new user for anonymous IIS access to the php bits of the site.
