Did you try:
extension_dir = d:/php/extensions
I still can't grasp it, but it seems to like complete paths with slash
(not backslash).

Is this the first time you install php on this machine? No previous
installations with some dll's lying around there somewhere?
Application error sounds like dll conflict. Is PHP as module? Probably
something with a duplicate msvcrt.dll?
