I got this problem some month ago. In my case it was caused the lack of some dll that I required
from php.ini.
In that period many e-mail was written about this problem, you may have a look at mailing list
archive and search for X-Powered-By.
Bye

Subject: [PHP-WIN] PHP4.0.3 Problems
Hi all,
I just set PHP4.0.3 up on Win98 with Apache 1.3.12. Whenever I load a php
page I ALWAYS get the following :
X-Powered-By: PHP/4.0.3 Content-type: text/html
at the top of the web page. This is the version of PHP4.0.3 from php4win.de
.
Can anyone say whether this might be due to my setup (most likely) or if its
a bug with PHP? Also, how do I correct it?
This is from my httpd.conf :
#LoadModule php4_module c:/php4/sapi/php4apache.dll
<Directory "C:/php4/">
AllowOverride None
Options ExecCGI
</Directory>
ScriptAlias /php4/ "C:/php4/"
AddType application/x-httpd-php .php3
AddType application/x-httpd-php .phtml
AddType application/x-httpd-php .php
AddType application/x-httpd-php .php4
Action application/x-httpd-php "/php4/php.exe"
Thanx,
Kevin
