Yes. CGI basically means a new instance of PHP, a new process, is invoked for every hit. In the ISAPI module, that's not the case - PHP becomes a part of the IIS process, and is alive for as long as the server lives. The difference between the two approaches is very evident. CGI is much, much slower for obvious reasons. However, if something bad happens, e.g. memory corruption, with CGI - at worse, one process would mess up. online users? =) While someone has complained about PHP 4.0 being extremely slow under Windows, we haven't been able to reproduce this behavior, and it appears to be more or less as fast as its UNIX cousins. However, in order to support an online community with hundreds of users, you won't be able to rely on the stability of the ISAPI module at this time. So, you'd have to use the CGI version, which will significantly higher the load on your server. All in all - you'd have to actually see if it does the job for you.
