Bingo! Thanks, Ermanno.
I had installed PHP 3.0 in a folder named "PHP3" and then installed PHP
4.0.4 in another folder named "PHP." I had then deleted the "PHP3" folder
and its associated registry entries.
The PHP 4-related registry entries were correctly pointed to the "PHP"
folder but Personal Web Server continued to look for the PHP program in the
deleted "PHP3" folder and returned the 500 error.
After pulling my hair out trying different variations of registry entries,
I simply renamed the new "PHP" folder to "PHP3" and everything works
perfectly. Someday, I'll probably rename the folder "PHP" and reinstall PWS
so it recognizes the new program location but for now I'm not messing with
success.
Thanks for your help.

I had the same problem. It seems like PWS remembers the directory where you
installed PHP. I had to reinstall PWS. Maybe there is a better solution,
but PWS has no MMC, and i couldn't find any configuration file to change.
