The fix was easy, as we'd guessed. It turns out we've
been taking the documentation too seriously, and/or not
understanding the Apache build process thoroughly enough.
The setup program in the /dl directory of the PHP distribution
states:
If you are compiling php as a module, you should also add
-rdynamic to the start of the EXTRA_LDFLAGS in apaches
Configuration file.
We did just that: we edited Apache's src/Configuration file.
However, we were using the APACI build procedure, which means
that src/Configuration isn't used -- src/Configuration.apaci
was used instead.
Jim Winstead pointed us in the right direction on this one.

If anyone is maintaining the /dl files these days, I think
you'll spare some people time if you update the setup
program so it recommends editing /src/Configuration.tmpl
and then re-running the configure script in the Apache
directory so that src/Configuration and src/Configuration.apaci
are re-generated.
Also, bug 1149 can be closed.
Our $150 reward will be sent, with Jim's blessing, to the
general PHP development fund.
