May I ask why you place the form into a varible then echo it?
Just write the code in normal html outside of the <?php ?> tags..
As for your varibles not being sent, I can only suggest that you have
upgraded your PHP recently to one above 4.1 and that you now have to use
$_POST['varible'] instead of $varible.
This change has been well documented in the php changelogs, and on this
forum...
If that change doesn't help you place a phpinfo(); line on your
submit_entry.php page, and it will show you were you values are ending up
