I don't think there is an "application/x-httpd-php4" mime-type. There's
just a "application/x-httpd-php" mime type, so delete the first
"AddType" line.

You could add the .html extension to the php mime-type, but I wouldn't
recommend it. I don't understand the need to put PHP and SSI together in
the same page. PHP can do everything SSI can.
