You can generate a PDF under PHP control, and print that from the browser.
Often overlooked as a printing solution is Flash 4 or 5. If you create a
Flash movie (.swf file) that represents the form, you can pass in variables
and then use the built-in printing capabilities of the Flash player. I've
used this for kid-oriented web sites with lots of graphics and it works
nicely. YMMV.
