protonic.com have developed exactly that system - tailored towards a free volunteer help-desk site w/MySQL. Ideally the package would work with email on the users send, send a message to support@domain.com and voila, it is in the system. Form based works fine as well. PHP based. Would be eternally grateful to projects like this out there on the net. Perhpas some others would be as well.
