http://pear.php.net/manual/en/developers.packagedef.php It is mandatory that you have such a file. The pear command relies on that file to install packages. Without it there is no way somebody can do a pear install phpdocumentor. As a side note (it is not stated in the spec) you can have <file dir="your-dir" install-as="install this file as" role="php">DAX.php</file> So you can change the way the directories are named in the cvs and when you install it.
