Hi,
I don't have shell access, but I have everything else I need, ftp, non-docroot area, etc. I know you might say "well get shell access" and I would be happy to, if you foot the bill. If I was the only person in the world with this situation, I wouldn't bring it up, but many of the hosts I investigated who advertise PHP support have this setup. PEAR would benefit from having this larger audience.
In any case, this and Pierre-Alain's answer should answer Martin's question. :)
I just took a peek at the registry (I used that registry file), and put 2 and 2 together (SYSCONF = conf, don't it). It would be wonderful if someone would document these things for the pear.php.net, but that's obvious :)

I'd like, for instance, to be able to install pear on the virtual host for chiaraquartet.net, but I have no root access, and they won't install PEAR globally. I know I could just upload an installed version from home, but this is a major pain for upgrading, lynx -source http://go-pear.net/ | php then setting install prefix to within my home directory did it for me. Upgrades are easy, just using the pear commmand in my install. Apache is taken care of by setting php_value include_path in .htaccess - cnb
