pear run-tests <package> This could be done as a default option, if they specify a path then the installer could take that into account. If you want to run test files, use run-tests.php, I don't see any point in simply wrapping run-tests with the pear installer. It should be used to test pear packages by default.
The point is that run-tests is for you as the developer, to make sure that your code works as expected, that you didn't introduce BC breaks with any changes or to create reproducable test cases for bug fixes. And you almost always want to do this in your development environment, e.g. in a CVS checkout. There is no point (well, at least not such a big point) in making it comfortable for users to run these tests on installed packages.
