There is no need to upgrade the entire PHP tree:
cvs -d :pserver:cvsread@cvs.php.net:/repository login
# Get Pear
cvs -d :pserver:cvsread@cvs.php.net:/repository co php4/pear
# Update Pear
cvs -d :pserver:cvsread@cvs.php.net:/repository update php4/pear
