Uploads with HTML_QuickForm_Controller

I am trying to find an example of uploading a file as part of a QFC submission. The form has other fields besides the file field.
The only example I found has a separate wizard page with an UploadAction class. This is not what I need.
WHat do I do inside my ProcessAction class to test for the existence of an uploaded file (which is optional) and if there, get access to it?
I tried the archives of this list with little success. Google doesn't seem to properly index our archives on the marc site.
