What do you want to use it for? As popular as the idea appears to be, I
find this an unintuitive control in just about every case.
I considered using this recently to set the order of menu blocks in a
web app I'm writing, and eventually decided to go with something like
this:
Active | Block Name |
[x] Users: New Users Move Up | Move Down
[ ] Users: Profile Move Up | Move Down
[x] Admin: Options Move Up | Move Down
Since I think users are more likely to understand the concept of moving
objects up and down, then adding and subtracting them from seperate
lists.

It's probably easier to create an element type... see
http://cvs.php.net/cvs.php/pear/HTML_QuickForm/QuickForm
for the current
element definitions, and work from there I guess.
