nice work. I have a few suggestions though. Question 1:

            Replace the input field with a textarea and add some default
            options with checkboxes, like [ ] heard from a colleague
            [ ] read about it at xy, and so on.

Question 5,6:

            Add checkboxes with the most common packages, a 5 x 8 or so
            would suffice for that.

Question 8:

            "Quality about PEAR advertising/marketing/evangelism"

Question 9:

            "enhancing docs" "enahncing code quality" ...

Question 11:

            add "[ ] plan to"

On page 4 there could be another question: "In which areas does you company use PHP: [ ] Web, [ ] Commandline, [ ] Other" and probably "Is your company doing OpenSource software?". Question 23: there is also PHP|Arch and the french one and a polish one. And a question about external PEAR (doc) resources might be of interest.
