Document is really good. I am sure this will help all new test writers.
One comment on description of --TEST-- section :
I would be nice to talk(through example) about writing --TEST-- section
which indicates whether test is a basic/variation/error or give few example
to say if the test is a basic test --TEST-- section would look like this,
etc. This will bring --TEST-- section inline with test generated by
generate_phpt.php
