I have posted this problem to this list and submitted bug report N 1947.
br@adviser.com got a workaround in bug report N 1948:
This report is related to Bug #1947
In basic_functions.c when compiled as an apache modul the file
ap_compat.h is included. This .h file contains the definition
#define md5 ap_md5 which results in not using the php md5.
Just put after the include of php.h an #undef md5 if compiled
as an apache module.
After those changes made in latest CVS md5 forks fine for me.
