Perhaps your mails aren't getting through?
Sorry, only kidding.
Here's something that has helped me in the past with sendmail after I get
really stuck... Sendmail takes an option "-X" that will dump all its traffic
and conversations to a file -- check the manual for details. So, if you can
restart your daemon with this option (it's *not* for the php mail command's
use) and you have space for what could easily be an enormous file on a busy
mail server, then try it. That way you'll see all and anything the mail daemon
hears, says, does, transmits, rejects, and so on.
