For namespace registration there is an additional approval that must come from the volunteer pool on the appropriateness/lack-of-redundancy of that namespace name. This is the same volunteer pool as above, though I believe (though I'm uncertain) that certain people attend to certain top-level namespaces. CPAN often gets maligned as a trash pit, but it has gotten much better. There is much less redundancy in new packages and the auto-tests are actually pretty thorough. George
