I had *exactly* the same problem so I know how you feel ;)
My problem was not with php4, but with apache and making sure DSO support
was correctly compiled
Try this:
cd /usr/local/apache
make distclean
./configure --enable-module=so --enable-shared=max
make
make install
cd /usr/local/php4
./configure --with-mysql --with-apxs=/usr/local/apache/bin/apxs
make
make install
