I'm in favour of having this in 4.0.7, and releasing 4.0.7. The main reasoning being that we already break lots of APIs with 4.0.7, we might as well move this in now as well.
The fact we haven't done anything about this for almost two years doesn't mean we should prevent someone that really wants to finally address this issue (Stig) from doing this. Having two structures is a possibility, but it's also a fairly big headache, so unless you volunteer - Stig's patch should be incorporated to the CVS, IMHO.

There is no reason to rush this change now. The debate about implementing more sensible error messages etc. for dynamic extensions dates back to the meeting in Tel Aviv 1.5 years ago; I cannot say that PHP desperately lacked this feature and that the lack has hindered PHP's success in the meantime.Instead I would suggest to consider adding some backwards compatibility on the source-level.
