What I wanted to say: Every '_' in the class name has to get a '/' in the directory structure (although I can't find this in the standards at the moment). If you want to put your class into HTML/QuickForm/, your class name would have to be HTML_QuickForm_LiveSearchSelect and the file name would have to be livesearchselect.php. (Just my interpretation, maybe somebody want's to correct me.)
