You still didn't provide any details why your additions need to be PHP 5.
What I would like though it the ability to have a test audience for this app and pear provides an easy distribution for that.And you would get an even bigger test audience if you keep it compatible with PHP 4. Net_SMS is really not rocket science that needs the latest and greatest OO/SPL/XML/etc support that only PHP 5 offers.

Though I can already promise you that this package won't be turned in to PHP5 anytime soon.
