Yes, there is a problem under windows with env vars.
Try a
pear config-set bin_dir c:/your-path-to-php
pear config-set doc_dir c:/your-path-to-pear-doc-sir
etc.
for info here is my conf

After the conf is right you can do pear install HTTP_Upload for example
