Just a bit background on this: The file has to have this name and
include the declaration for a phpext_{$name}_ptr not only due to coding
standards but for being able to build the extension statically into PHP
as the build system will include it from
main/internal_functions[_cli].c.
That's also the reason why that header shouldn't contain private
elements which might conflict with stuff included by one of the other
headers there.
