I know, but I'm not talking about the future of the "pear" cmd, I'm
talking about its present, where the CGI version > 4.1.x is supported (I
use it for writing it!).

Have you ever tried it? FYI the -C and -q params are still preserved for
BC issues. Your problem was with the FreeBSD OS not allowing such
interpreter, nothing related to PHP.
